Before you call, carefully read the following:

If you decide to request a personalized plate combination, you must explain what the combination means or what the combination represents. The DMV reviews requested personalized plate combinations. The DMV is not required to issue a requested plate. You can read the DMV regulations for restrictions on personalized plate combinations.
You must have a current New York registration to order a custom plate. Have your current registration information ready when you call. Refer to your registration receipt or window sticker (see samples).
You can pay for your plates using one of the following credit cards:
- American Express®
- Discover Card®/Novus®
- MasterCard®
- VISA®
Your new registration and window sticker will be mailed first. Allow at least 10 days for the documents to arrive. Your new plates will be delivered by mail in approximately 4- 8 weeks.
All items will be mailed to the address on your registration. If you have moved since you last registered your vehicle, see Name or Address Change on our FAQs page.
If you order a personalized plate, write down your combination and have it ready when you call. If you choose not to personalize your custom picture plate, we will assign the next available plate combination to you.
Some plates are not available by phone. Professional, emergency services and some organization plates are limited to membership and must be ordered through the organization or directly from our Custom Plates Unit, with documentation.

Orders by Mail:

Mailbox image To order your plates by mail, use form CP-62.1 (Order Form for Personalized Plates or Cause, Organization, Regional, Colleges and Fraternities or Sports Picture Plates). You need Adobe Acrobat Reader® to download the form.

Mail the completed form and your payment to the address shown on the form.

When you calculate your fees for a mail transaction, remember that the date you mail the form is not the date the transaction is processed. See the fee tables above for prices by the date the transaction is processed.

Some Organization plates cannot be purchased by mail.

Email orders cannot be accepted.
