Please consider enrolling in the New York State Organ and Tissue Registry.

To enroll in the New York State Organ and Tissue Donor Registry, check mark the "Organ Donor" box on your Application For Driver License or Non-Driver ID Card (MV-44) or your application to Renew Your Driver License By Mail (MV-2L) or Renew Your Non-Driver Identification Card By Mail (MV-2ID), or contact one of the organ procurement organizations listed on this page.

The DMV will forward your information to the State Department of Health to include your name in the Registry.

If you are not applying for or renewing a license or non-driver ID card, and you want to consider becoming an organ or tissue donor, contact the New York State Department of Health or an Organ Procurement Organization.
