To enroll in the New York State Organ and Tissue Donor Registry, check mark the "Organ Donor" box on your Application For Driver License or Non-Driver ID Card (MV-44) or your application to Renew Your Driver License By Mail (MV-2L) or Renew Your Non-Driver Identification Card By Mail (MV-2ID), or contact one of the organ procurement organizations listed on this page.

The DMV will forward your information to the State Department of Health to include your name in the Registry.

If you are not applying for or renewing a license or non-driver ID card, and you want to consider becoming an organ or tissue donor, contact the New York State Department of Health or an Organ Procurement Organization.

• Your decision to donate will not affect your medical treatment.
  
  
• The organs and tissues you donate will go to patients in the greatest need.
  
  
• There is no cost to the donor's family.
  
  
• Make sure your wishes are fulfilled. Tell your family you have enrolled in the Registry.
  
  
• You can have your name removed from the Registry at any time.
  
  
• After you sign-up, the NYS Department of Health will mail you a confirmation letter to provide you with additional information.
